This Initial Consultation Agreement & Disclaimer ("Agreement") is entered into by and between Elemental Collaborative, LLC ("Provider") and the undersigned individual ("Client"). This Agreement governs a single, one-hour consultation session only.
Scope of Consultation.
The consultation is limited in scope and intended solely to provide general, peer-based guidance and discussion related to recovery-oriented support and care navigation. The consultation may include listening to Client concerns, offering non-clinical perspectives, discussing potential support options, and assessing whether a mutual fit exists for potential future services. The consultation does not create any ongoing obligation or continuing relationship unless and until a separate written service agreement is executed by both parties.
Non-Clinical Disclaimer.
Provider is not a licensed therapist, psychologist, physician, or medical provider. The consultation does not constitute therapy, medical treatment, diagnosis, legal advice, or crisis intervention. Client remains solely responsible for all decisions and actions.
Confidentiality.
Provider will maintain reasonable confidentiality regarding information disclosed during the consultation, except where disclosure is required by law or necessary to prevent imminent harm.
Emergency Notice.
This consultation is not intended for emergencies or crisis situations. If immediate help is needed, Client agrees to contact emergency services (911) or a licensed healthcare provider.
Fee & Payment.
Client agrees to a consultation fee of $300. The fee shall be invoiced following completion of the consultation and is payable upon receipt using Client's preferred payment method.
No Guarantee of Future Engagement.
Completion of the consultation does not obligate either party to enter into any additional services or agreements.
Limitation of Liability.
To the maximum extent permitted by law, Provider's liability arising from the consultation shall be limited to the consultation fee paid.
